HaysMed Conducts Community Health Needs Assessment
HaysMed To Conduct
Community Health Needs Assessment Survey
January 30 – February 15
Hays, Kansas (January 24, 2012) HaysMed is working with other Ellis County Kansas health providers to conduct a community health needs assessment. Results of the survey will be compiled and prioritized to help guide the hospital's plans to address health care needs and fulfill both federal and state requirements.
Starting the week of January 30th, surveys will be mailed to 2000 randomly selected Ellis county residents. If you do not receive a survey in the mail and want to give feedback for this assessment go to www.haysmed.com/communitybenefit to complete the survey online. Copies of the paper survey will also be available at HaysMed, Monday through Friday from 8:00 am – 5:00 pm, at the Information Desks in the Miller Medical Pavilion Entrance B and Main Entrance C and at The Center for Health Improvement. Surveys must be completed and returned by Wednesday February 15th, 2012.
Included in the survey are questions dealing with health related topics such as diet/nutrition, exercise, health concerns, and access to care. All survey responses are anonymous and completing the survey takes less than 10 minutes. VVV Research & Development, Inc. an independent research firm from Olathe, Kansas has been retained to conduct this county wide research.
