Tuition Reimbursement Program at HaysMed

Eligibility for Participation
All associates that have completed six (6) consecutive months of employment are eligible to apply for tuition reimbursement. Full time associates are eligible for tuition reimbursement up to $1,500 per fiscal year; part time associates are eligible for tuition reimbursement up to $1,000 per fiscal year; and PRN associates are eligible for tuition reimbursement up to $600 per fiscal year.

Criteria for Approval
Courses must be: a) directly related to the associate's current position or a position for which Hays Medical Center anticipates immediate or future staffing needs to which the associate can progress, b) part of a planned degree program in a field applicable to Hays Medical Center, c) taken on the associate's own time and d) taken at approved institutions including accredited colleges and universities, and vocational/technical schools.

Administration
An application for each course must be completed, signed and approved and submitted to the human resources department along with the proof of tuition and book fees and completion proof/grade. Approval for tuition reimbursement is contingent upon available funding and relevance of the course to your planned degree. Your manager, Vice President and Human Resources representative will determine whether the course meets the criteria for approval.

An applicant must obtain the required approval and submit all materials (application, proof of course fees and books and completion proof/grade to the human resources department. You bear the cost of items including but not limited to: equipment, supplies, parking, application costs, lab fees, test fees, transcripts, late fees, copy costs, insurance, social/activity fees and society/club dues.

You will be reimbursed for approved expenses if you attain a grade of C or higher, or a passing grade if graded on a pass/fail basis, when you submit receipts for covered expenses and an original grade report to the human resources department within (6) weeks following completion of the course. Federal law permits tax-free reimbursement for undergraduate courses.

If your employment terminates within six (6) months after receiving tuition reimbursement you are required to repay the amount of the reimbursement in full. By signing this application, you agree to authorize a payroll deduction from your final payment of wages and paid time off (PTO), so Hays Medical Center can recover its share of payments made to you, in the event you do not complete your work commitment following completion of the course.

To be considered for education assistance in the form of tuition reimbursement, please complete the application on the reverse side and forward to your manager to start the approval process. If your application is denied, it will be returned to you with an explanation of denial..
