If you are interested in enrolling in the Diabetes Solutions Class ask your primary provider to fill out the referral form, sign it and fax it back to us. Our staff will then contact you.

Our Mission
To provide quality, comprehensive diabetes self-management education. We believe that education is the key to empowering the individual with diabetes to better manage their disease. Diabetes Solutions is accredited by the American Diabetes Association.
It takes the support of a diabetes team (primary provider, diabetes educator, dietitian, exercise physiologist, behavioral health advisor, care partner, etc.) to achieve optimal control of their diabetes.
Our Goals
For participants to become active in their own care and diabetes management
To provide avenues to be successful in achieving self-management goals
The program will focus on the national standards of care according to the AADE and ADA
To provide interactive learning and up to date information on diabetes self management
To provide education to the caregivers so they feel comfortable in being a part of the support team (Care partners are encouraged to join patients at no additional charge.)

A comprehensive program which consists of two, 5 hour sessions. (see class schedule)
This is a comprehensive program which consists of two, 5 hour sessions. The program is accredited by the American Diabetes Association.
Each session is five hours and will run from 8:45 a.m. to 2:30 p.m. and includes lunch. It is not required that you do the consecutive classes in the same month. Classes are held in the St. Joseph Meeting Room at Pawnee Valley Campus, 923 Carroll Avenue, Larned KS 67550.
Also available:
Private consultation with a certified diabetes educator for diabetes education, insulin starts, medication adjustments, pump starts, etc.
One on one consultation with a dietitian for medical nutrition therapy
Gestational diabetes consultation.
Gestational Diabetes
Are you pregnant and have just found out that you have gestational diabetes? Or, are you a diabetic who is now pregnant or trying to conceive? We can help! We feel very strongly that the more knowledge you obtain about diabetes, the more involved you will become in your diabetes management. This in return will help you gain better control. We offer individual consultation with Andrea Paul, PA-C, who provides medical education about Gestational Diabetes and formulates a treatment plan specific to your needs. You will also meet 1:1 with a registered dietitian to receive information about dietary recommendations. The following topics will be discussed:

• Goals of Medical Nutrition Therapy for Gestational Diabetes
• Dietary Guidelines for Gestational Diabetes
• Pathophysiology of Gestational Diabetes
• Maternal/Fetal Effects
• Signs, Symptoms, Testing, and Treatment
• Labor, Delivery, and Postpartum
• Glucose Monitoring
• Insulin Administration
• Exercise
Getting in for a visit is easy: ask your OB/GYN or primary provider to fill out the referral form, sign it, and fax it back to us. Our staff will take care of contacting you.
Education is offered by individual consult only with a referral from your doctor.
